Decription of the Professional Practice Gap (PPG)

Evidence has shown that inadequate patient history taking, and communication adversely affect patient care and can lead to bad outcomes. This activity will seek to improve patient outcomes by improving providers’ communication and history taking skills.

Learning Outcome Statement

80 percent of all participants will indicate an intent to make at least one change in practice related to the review of the medical history as a result of attending this program.

Learning Objectives

After completing this activity, learners will be able to:

  • Establish a process for reviewing patient records, including patient history, at the initial visit and subsequent encounters.   
  • Explore underlying reasons why a patient may not provide a complete medical history and address barriers by implementing communication strategies. 
  • Evaluate key communication issues between providers during transition of care that can lead to poor patient outcomes. 
  • Assess my documentation methods for informed consent and implement new strategies to ensure all elements of informed consent are adequately documented.
