Prevention of Medical Errors (2023–2025 Edition)

Description

Few medical errors are attributed to faulty medical judgment. More commonly, they are attributed to system failures inherent in healthcare delivery. By drawing on our professional liability closed claims data, we have identified common performance and diagnostic errors. We analyzed loss prevention measures in tandem with the elements necessary to conduct a credible and thorough root cause analysis to reduce system failures, respond to sentinel events, prevent medical errors, and improve patient safety. The purpose of this program is to provide clinicians with the most current information regarding the prevention of common performance and diagnostic errors.

Desription of the Professional Practice Gap (PPG)

Healthcare and medicine are not static fields. The pace of technological advances, evidence evidence-based literature release, and regulatory changes is increasing. As healthcare delivery becomes more complex, so do the risks. Periodic evaluation of current patient safety information, such as the most common adverse events identified by regulatory agencies and closed professional liability contributing factor data is necessary for providers to address patient safety in their practice settings.


Learning Outcome

The learner will apply risk mediation strategies within three months of completing the program, for the purposes of reducing the risk of medical error and increasing patient safety in their practice setting.


Learning Objectives

After completing this activity, learners will be able to:

  • Assess my practice to identify opportunities to reduce the risk of medical errors.
  • Implement at least one change in my practice that can reduce the risk of medical error.
  • Educate my staff and colleagues about specific changes in policies and/or processes that can reduce the risk of medical error
