Program Description

The Inpatient Care Management webinar will address healthcare updates and trends related to accountable care, clinical integration, and the functions of inpatient CM including utilization management, concurrent review, and discharge planning and how inpatient care management Is evolving.

Objectives

After completing this activity, learners will be able to:

  • Examine the goals, roles, and responsibilities of inpatient CM.
  • Identify the various inpatient CM models and the evolution of inpatient CM.
  • Express how effective CM benefits the patient.

 

Program Description

The Interdisciplinary Care Teams webinar will address the objectives of an interdisciplinary care team model and importance of communication and collaboration across the continuum among different providers and teams.

Objectives

After completing this activity, learners will be able to:

  • Demonstrate how the new model of interdisciplinary teams differs from the old model.
  • Recognize the different types of interdisciplinary team rounds.
  • Identify challenges with conducting interdisciplinary team rounds.
